Clozapine vs Olanzapine

How clozapine and olanzapine compare on uses, side effects, and what to expect.

How they're similar

Clozapine and olanzapine share more at the mechanism level than their different clinical roles might suggest. They're both atypical antipsychotics and they share several important features.

  • Both block dopamine D2 receptors and serotonin 5HT2A receptors, though clozapine has weaker D2 binding and hits a wider set of other receptors, including muscarinic, histaminergic, and adrenergic ones.
  • Both are FDA-approved for schizophrenia.
  • Both carry the class boxed warning about increased mortality in elderly patients with dementia-related psychosis.
  • Both are among the worst offenders for weight gain and metabolic effects in the atypical class. Both can cause substantial weight gain, increased blood sugar, and dyslipidemia.
  • Both require metabolic monitoring, including weight, blood pressure, fasting glucose, and lipids at baseline, at three months, and then at least annually.
  • Both can cause sedation, especially at higher doses and early in treatment.
  • Both are available as generics.
  • Prophylactic use of metformin has been shown in several studies, including work by Carolan and colleagues and others, to attenuate the weight gain that comes with either drug. Some clinicians start metformin at the same time as clozapine or olanzapine in patients who are metabolically at risk. Weight-focused counseling, exercise, and diet support are also standard parts of care.

The overlap on metabolic risk is worth taking seriously. Both drugs sit at the top of the metabolic burden ranking, and the monitoring and lifestyle strategies for both are essentially the same. What separates them is the additional monitoring clozapine requires and the additional side effects it carries.

How they differ

The differences between clozapine and olanzapine are wide. Clozapine has efficacy that olanzapine can't match in treatment-resistant cases and has an anti-suicide indication that olanzapine doesn't share. It also comes with a set of serious warnings, monitoring requirements, and side effects that olanzapine doesn't have. The table below summarizes the main points, with detail underneath.

Clozapine is reserved for treatment-resistant schizophrenia. The standard definition is that a patient has failed at least two adequate trials of other antipsychotics, meaning they've been on those drugs at a therapeutic dose for a long enough time to know they didn't work. For that group, clozapine has efficacy no other antipsychotic matches. This isn't a minor difference. In treatment-resistant illness, clozapine can produce responses that other drugs haven't, and it's the standard of care in that setting.

Clozapine also has a specific FDA approval for reducing suicidal behavior in patients with schizophrenia or schizoaffective disorder who are at high risk. No other antipsychotic carries that indication, and the evidence supporting it comes from a specific trial (the InterSePT study) that compared clozapine to olanzapine directly. For a patient with schizophrenia and a history of serious suicidal behavior, that indication can be decisive.

The safety burden that comes with clozapine is substantial. Agranulocytosis, meaning a severe drop in neutrophils that leaves a patient vulnerable to serious infection, occurs in about 0.4 to 0.8% of patients on clozapine. The risk is highest in the first six months of treatment. Because of this, patients on clozapine need absolute neutrophil count (ANC) monitoring. The traditional schedule is weekly for the first six months, biweekly for the next six months, and monthly thereafter. Until 2025, this monitoring was enforced through a formal REMS program that required prescribers, dispensing pharmacies, and patients to be registered. The FDA eliminated the REMS in 2025 because the program was seen as a barrier to appropriate use, but the monitoring itself is still standard of care. Prescribers still check ANCs on the same schedule.

Clozapine also carries other serious warnings. Seizures are dose-related and become more likely at higher doses (above 600 mg per day is a particular concern), which is one reason clozapine is titrated slowly. Myocarditis, meaning inflammation of the heart muscle, is a rare but serious risk, especially in the first eight weeks of treatment. Symptoms include chest pain, fatigue, shortness of breath, and fever, and any of those in a patient starting clozapine warrants immediate evaluation. Orthostatic hypotension is common and requires a slow titration to reduce falls and syncope.

Constipation is a serious concern with clozapine, more so than with most other psychiatric medications. Clozapine's strong anticholinergic effect can slow the gut significantly, and the constipation can escalate to ileus, meaning a complete or near-complete shutdown of intestinal movement. Ileus on clozapine has caused deaths. Standard care includes prophylactic bowel regimens for many patients, and clinicians ask directly about bowel movements at every visit.

Hypersalivation, meaning excessive drooling especially at night, is another common clozapine side effect that patients often find unpleasant. It usually improves with time but sometimes needs specific management.

Olanzapine's monitoring is simpler. It requires only the standard metabolic monitoring: weight, blood pressure, fasting glucose, and lipids. No blood counts. No specific cardiac monitoring beyond baseline. Titration is faster. In inpatient settings, olanzapine can be brought to a therapeutic dose within days, while clozapine typically takes weeks.

Side effect tendencies

Both drugs cause substantial metabolic effects, and prophylactic metformin has been studied and used for both. In practice, clozapine's metabolic effect is at least as bad as olanzapine's, and some patients gain even more weight on clozapine. Fasting glucose and lipids often move in the wrong direction on either drug. Weight gain of 20 pounds or more in the first year isn't unusual on either.

Sedation is significant with both. Clozapine tends to be more sedating overall, which can be useful in acutely ill patients but hard to work through long-term. Olanzapine's sedation often eases somewhat over the first weeks.

Clozapine has the additional layer of side effects that olanzapine doesn't share to the same degree: hypersalivation, severe constipation, orthostatic hypotension, seizure risk at higher doses, myocarditis, and the neutropenia risk that drives the ANC monitoring. Olanzapine has less of each of these.

Movement side effects, or EPS, are low with both. Both are considered low-EPS agents, and clozapine in particular has essentially the lowest EPS risk of any antipsychotic in current use. Clozapine is often chosen for patients who've had severe EPS or tardive dyskinesia on other antipsychotics, because it's much less likely to cause or worsen those effects.

Both drugs can cause dry mouth, blurred vision, and other anticholinergic effects. Clozapine's anticholinergic burden is heavier than olanzapine's, which contributes to the constipation, urinary retention, and cognitive slowing that some patients experience.

What tips the choice

The decision between clozapine and olanzapine is usually straightforward when the clinical situation is clear. Olanzapine is for a wider set of patients, and clozapine is for a specific subset.

A clinician chooses olanzapine when the patient hasn't yet had adequate trials of first-line antipsychotics, when acute psychosis or mania needs faster titration, when the treatment target includes bipolar disorder, or when the patient can tolerate the metabolic burden and the sedation. It's a common first or second choice for many people with schizophrenia and for many with bipolar mania.

A clinician chooses clozapine when the patient has treatment-resistant schizophrenia, meaning at least two other antipsychotics have failed to produce adequate response after full trials. Clozapine is also considered when a patient with schizophrenia has serious suicidal behavior, because of its specific FDA approval for that indication. It's the drug of choice for patients with severe tardive dyskinesia or Parkinson's disease-related psychosis, because it doesn't worsen movement problems.

The barriers to clozapine are real. The blood monitoring, even without the formal REMS, requires regular blood draws that some patients can't or won't do. The side effect burden requires attentive management. The slow titration means clozapine isn't useful in acute situations where rapid symptom control is needed. And clozapine's specific serious risks, including myocarditis and ileus, require patient education and access to care if symptoms come up.

For a patient with treatment-resistant schizophrenia who's been through several other drugs without adequate response, clozapine is often life-changing, and delays in starting it are common but usually unhelpful. For a patient who hasn't had that trial history, olanzapine or another first-line antipsychotic is usually where treatment starts.

Common questions

Why is clozapine considered the strongest antipsychotic if it isn't first-line? Clozapine has the best evidence for treatment-resistant schizophrenia, where other antipsychotics haven't worked. Its efficacy in that specific setting is unmatched. But its side effect and monitoring burden is heavy enough that it isn't a reasonable first choice for most patients. First-line drugs work well for many people, and starting them first means most patients can avoid clozapine's monitoring and side effects. When first-line drugs don't work, clozapine is the next step, and delays in getting there often reflect the practical barriers of monitoring rather than a real clinical question about whether it should be tried.

Do I still need weekly blood work if the REMS is gone? Yes, effectively. The FDA eliminated the formal REMS program for clozapine in 2025 because the program was seen as a barrier to appropriate use, not because the monitoring itself was unnecessary. The monitoring schedule (weekly ANC for the first six months, biweekly for six months, monthly after that) is still the standard of care, and prescribers still follow it. The change is administrative rather than clinical. If a lab draw is missed or an ANC drops, the response is the same as before.

Can metformin actually prevent weight gain on these drugs? There's real evidence that metformin, when started at the same time as clozapine or olanzapine, can reduce the amount of weight patients gain, sometimes substantially. Studies by Carolan and others have shown this in both drugs. Metformin isn't a full solution, and it doesn't work for everyone, but it's a reasonable option to discuss with a prescriber, especially for patients who are already overweight or have a family history of type 2 diabetes. It doesn't replace diet, activity, and monitoring.

What's the biggest safety concern with clozapine? The one that gets the most attention is agranulocytosis, because it can be life-threatening and it's the reason for the ANC monitoring. But in day-to-day care, constipation and ileus are also serious concerns, and prescribers usually ask about bowel movements at every visit. Myocarditis is rare but serious and mostly occurs in the first eight weeks of treatment. Seizures are dose-related and become more likely at higher doses. Orthostatic hypotension and the falls that can come from it are also common early problems. Each of these has a specific management plan.

Is olanzapine ever tried before clozapine? Yes, almost always. Standard practice is to try at least two other antipsychotics, often including olanzapine, before moving to clozapine. That's what defines treatment-resistant schizophrenia and what leads to clozapine. If a patient hasn't had that trial sequence, clozapine usually isn't the next step. There are situations where clozapine is considered earlier, especially in patients with a strong suicidal history in schizophrenia or with severe tardive dyskinesia, but for most, olanzapine or another first-line drug comes first.
