Aristada Initio: rapid LAI initiation for aripiprazole

The problem it solves

Traditional aripiprazole LAI initiation:

• Abilify Maintena: 14 days of oral aripiprazole overlap
• Aristada: 21 days of oral aripiprazole overlap
• Patient must reliably take oral aripiprazole daily during this period
• Non-adherence during overlap is a common reason for LAI treatment failure
• Hospital discharge planning often requires transitioning to LAI at discharge, but oral overlap becomes barrier

Aristada Initio provides therapeutic aripiprazole levels within days rather than requiring weeks of oral overlap.

Protocol

Same-day initiation (Day 1):

• Aristada Initio 675 mg IM (single injection)
• Oral aripiprazole 30 mg (single dose)
• No further oral aripiprazole needed

Day 8 or later (Day 8 for Aristada, Day 15 or later for Abilify Maintena):

• Start regular Aristada 441 mg, 662 mg, 882 mg IM every 4 weeks (or higher dose intervals)
• Or start Abilify Maintena 400 mg IM every 4 weeks

Clinical scenarios

Hospital discharge to LAI: Inpatient starts Aristada Initio on discharge day plus oral aripiprazole. First maintenance Aristada dose given at outpatient follow-up in 1 to 3 weeks. No adherence gap.

Emergency department transition: Patient with schizophrenia presenting with acute exacerbation or non-adherence. Aristada Initio protocol can start LAI treatment before discharge.

Outpatient initiation for adherence-challenged patients: Patients with prior oral treatment failure due to adherence. Aristada Initio removes the requirement for reliable 14 to 21 day oral overlap.

Pharmacology

Aristada Initio uses smaller aripiprazole lauroxil particles that dissolve faster, producing therapeutic aripiprazole levels within days. The single 30 mg oral dose provides immediate coverage during the first few days.

Aripiprazole lauroxil particle size:

• Aristada Initio: smaller particles, faster dissolution
• Aristada: standard particles, slower dissolution for monthly (or longer) dosing

Same pharmacology as oral aripiprazole once absorbed.

Considerations

Injection site:

• Aristada Initio: deltoid muscle
• Regular Aristada: gluteal or deltoid depending on dose

Adverse events: same profile as aripiprazole. Akathisia, weight gain (modest), sedation, nausea, injection site reactions.

Not for treatment naive to aripiprazole: Patient should have tolerated oral aripiprazole first. Aristada Initio is for LAI initiation, not for testing tolerability.

Cost: Aristada Initio is more expensive than a course of oral aripiprazole covering the loading period. Cost-effectiveness argument is that improved adherence downstream reduces hospitalization.

Common questions

Does Aristada Initio replace oral aripiprazole? For the initiation period, yes. The single 30 mg oral aripiprazole dose plus the Aristada Initio injection covers the first several days until regular Aristada or Abilify Maintena starts.

How long does Aristada Initio last? Provides therapeutic levels for approximately 8 to 30 days depending on formulation and patient factors. Enough coverage until regular monthly LAI is started.

Can Aristada Initio be used for Abilify Maintena initiation? Yes. Protocol allows starting Abilify Maintena at day 15 or later after Aristada Initio.

What if the patient misses the first regular Aristada dose? Follow the missed dose protocol for the specific LAI. Aristada Initio does not protect indefinitely.

Is this appropriate for a patient never on aripiprazole? No. Patient should have tolerated oral aripiprazole first to establish that the drug is acceptable. Aristada Initio initiation to LAI is for patients already known to tolerate aripiprazole.

How does this compare to LAI initiation for other drugs? Paliperidone (Invega Sustenna) has its own rapid loading protocol (2 loading doses days 1 and 8). Risperidone (Risperdal Consta) still requires 3 weeks oral overlap. Aristada Initio is unique in providing single-injection rapid initiation.
